Le dispositif de formation se décline en 3 étapes : 1- Studio individuel domotisé (bâtiment collectif) 2- Appartement domotisé en Centre Ville de Chinon 3- Installation dans un appartement aménagé, dans une autre ville que Chinon, dans la France entière, en fonction du souhait du client Dans ce cadre, vous intégrez une équipe de 4 ergothérapeutes, et travaillez au sein d'une équipe pluridisciplinaire engagée, composée de conseillères en économie sociale et familiale (CESF), formatrices en insertion sociale (FIS), et aides-soignants. Vos Missions principales, en tenant compte du projet pédagogique personnalisé de nos clients (P2P) : - Evaluer le degré d'autonomie du client accueilli pour élaborer un diagnostic ergothérapique. - Préconiser, concevoir, organiser, réaliser et adapter des aides techniques, humaines, et environnementales afin de favoriser l'indépendance et l'autonomie du client dans son environnement. - Former, favoriser et développer les compétences et facultés d'adaptation ou de compensation du client. - Suivre les démarches liées aux installations à domicile : dossier PCH aide technique, aménagement, suivi des travaux en lien avec artisans, architectes, bailleurs.
